Request an appointment to visit with a Certified Child Passenger Safety Seat Technician utilizing the Request Appointment Form above.

Once we receive the Request form with all of the necessary information, we will reach out to you to schedule an appointment.

In the message section of the Request Appointment Form, please include the Make and Model of the Child Passenger Safety Seat. 

On the day/time of the Scheduled Appointment, please bring the Child Passenger Safety Seat and Child (If available). Please plan for the Scheduled Appointment to take 1 to 1 1/2 hours to complete.
